Cooper found that distance covered maps linearly onto measured VO2 max, which gives the formula this calculator uses: VO2 max equals (meters minus 504.9) divided by 44.73.
The number is only as honest as the effort. It should feel like a hard, even effort from the first minute to the last, not a sprint-and-die. Runners who blast the first lap and fade cover less total distance than they would at a steady hard effort, and the formula reads that shortfall as lower fitness. Settle into the fastest pace you believe you can hold for the full 12 minutes, then fight to hold it.
Why the military still uses it
More than five decades later, the Cooper test survives because it needs no lab, scales to groups of any size, and correlates well with directly measured VO2 max. One officer with a stopwatch and a marked track can test an entire unit in a quarter hour. That practicality is why many militaries, police academies, and soccer clubs still use the 12-minute run as a standard fitness gate: it is objective, hard to game, and cheap to repeat. Worked example: 2,400 meters in 12 minutes
Say you cover 2,400 meters, six laps of a standard track, in your 12 minutes. The formula gives (2400 minus 504.9) divided by 44.73, which is a VO2 max of about 42.4 ml/kg/min. For a 35 year old man, the norms for ages 30 to 39 put good at 42 and excellent at 47, so 42.4 classifies as Good. If a season of training later he covers 2,600 meters on the same track, his estimate climbs to 46.8, knocking on the door of excellent. That is the test working as intended: a repeatable number that moves when fitness moves.

How often should I retest?
Every 8 to 12 weeks. VO2 max adapts on the scale of months, so retesting sooner mostly measures day-to-day noise. Is 12 minutes of hard running safe for beginners?
Not right away. The Cooper test is a maximal effort, and new runners should build a base of several weeks of easy running before attempting one.
